  
Families asking to
be considered for need-based grants must submit the following
information with the Enrollment/Re-enrollment Invitation(s):

A completed and signed Holy Spirit Preparatory
Need-Based Financial Aid Application. The application can be
found on the Holy Spirit Preparatory School website.
Alternatively, a paper application can be
obtained from Kelly Sickafoose at
ksickafoose@holyspiritprep.org.
- If you have not already submitted your 2006 tax return,
schedules and W-2’s, please include these documents when
submitting the application for need-based financial assistance.

If there is a 2007 net profit/loss from a business
reported on the need-based financial aid application, a Business
Statement must be completed. This can be found on the Holy Spirit
Preparatory School website. Alternatively, a
paper application can be obtained from Kelly Sickafoose at
ksickafoose@holyspiritprep.org.

If the student’s biological parents are divorced,
separated or unmarried both parents must complete the Holy Spirit
Preparatory Need-Based Financial Aid Application.

Families who have been awarded an estimated Need-Based
Grant must submit their 2007 tax return, schedules and W-2’s
before financial assistance can be credited to your family’s account.
Please note, if there is a significant discrepancy in the figures
provided on the Need-based Financial Aid Application and your actual
2007 tax return, your aid may be altered.

The school may request verification of actual income
for the school year for which the aid is granted.
All paperwork should
be included when submitting your Enrollment/Re-enrollment Invitation(s).
If your 2007 tax return is not complete at the time of
application, please send a copy to:
