UPPER SCHOOL SPIRITUAL RETREATS

Name & Date of Field Trip: Upper School Retreat

Students should bring: Bible, clothes appropriate for Mass, clothes for outdoor activities, closed toe shoes or tennis shoes, extra changes of clothes, a jacket or sweatshirt appropriate for the time of year, towel, sleeping bag, pillow, personal bathroom items, sunscreen and bug spray in warmer months. NO IPODs, video games or other similar electronic devices. Students may bring a cell phone, but all cell phones will be collected and kept by one of the chaperones upon arrival at the retreat site. Students may call home at any time, by requesting the phone. Parents will have contact numbers of the chaperones on retreat to reach their children at any time they wish. If any parents are uncomfortable with this rule, they may request an exception for their child by emailing Heather Triggs at htriggs@holyspiritprep.org 

***All high school students are required to participate in two retreats per year.

I hereby give permission for my child listed above to attend to attend the Retreat field trip on the scheduled (or any rescheduled) day(s) and to be transported by school bus, private bus, public transportation, van or private vehicle(s) as determined by the school. For myself and the named child; I hereby waive and release any claim against Holy Spirit Preparatory School and its board, employees, servants and agents for any injury or loss incurred by my child during the field trip (save for any personal injury directly resulting from gross negligence on the part of the school) and agree to indemnify the school against any expenses, loss or damages incurred as a consequence of any action or inappropriate inaction on the part of my child. I confirm that there are no medical or other circumstances that should be known to the chaperone(s) that are not noted in the Medical Conditions or Special Notes box above.
