Name of Field Trip: 2012 – 6th Grade Class Trip – Skidaway (Savannah, GA)

Date of Field Trip: Sunday, May 6, 2012 – Wednesday, May 9, 2012

Description & Objectives:
 The 6th grade field trip to Skidaway will provide students with exciting opportunities for exploration and discovery of dynamic ecological processes. This experience will increase student knowledge through hands-on activities and field studies that require careful observation, teamwork, and critical-thinking skills. This field trip will help students develop an appreciation of the coastal marine environments in the state of Georgia and foster respect for their beauty and complexity.

Chaperones: Travel and all activities in and around Savannah, GA will be supervised by Holy Spirit faculty members and administrators: Brian Moscona, Linda Anthony, Andrea Cruz, Norm Livermore, Fr. Jason Brooks, Tommy Curtin, Mr. Bob Pelletier,  Angela Krupczak (US Student Success Director), Lance Higdon (Dean of Student Life), Jane Sloan (Dean of Student Life), Pete Radosta (Dean of Junior High), and Karen Jimenez (Junior High Science).

Estimated Time of Departure: 3:00 PM Sunday, May 6, 2012

Estimated Time of Return: 3:00 PM Wednesday, May 9, 2012

Cost: $295.00 per student. Due to the contractual nature of large group travel, once we receive a deposit for each student we cannot provide refunds.

I hereby give permission for my child listed above to attend to attend the Class Trip to Skidaway (Savannah, GA) on the scheduled (or any rescheduled) day and to be transported by plane, school bus, private bus, public transportation, van or private vehicle(s) as determined by the school. For myself and the named child; I hereby waive and release any claim against Holy Spirit Preparatory School and its board, employees, servants and agents for any injury or loss incurred by my child during the field trip (save for any personal injury directly resulting from gross negligence on the part of the school) and agree to indemnify the school against any expenses, loss or damages incurred as a consequence of any action or inappropriate inaction on the part of my child. I confirm that there are no medical or other circumstances that should be known to the chaperone(s) that are not noted on the reverse of this permission slip.
